The god of cruelty, suffering, and insanity manifests in various forms in the dreams and visions of mortal creatures: a naked man of the viewer’s race with torture wounds, appearing melted, misshapen, and aberrant, with things like writhing tentacles and small, gibbering mouths all over his body, a torture wrack and a gag, the sound of someone being whipped or the sound of vomiting, or a massive, weeping sore.

The Mad One is greed, torture, malice, disease, criminal insanity, burglary, indifference, apathy, self-service, dishonor, disloyalty, heartlessness, expediency, betraying friends, lust for power, using others for your own ends, pain, fighting dirty, wounds, sores, crippling injuries, blindness, deafness, muteness, egotism, losing your mind, starvation, thirst, narcissism, and the lack of guilt or remorse. He blesses those who hurt others for their own gain, those who torture for any reason, and those who spread filth, disease, and suffering. He is worshiped by executioners, torturers, plague doctors, criminals, and any who betray, use, and spread misery to others.

His priests are called Cruel Ones, his clerics called Torturers, and his paladins Plague Bringers. His favored beer is rauchbeer. He lives on Gehenna in a domain called the Dungeon.
